Community Support Sercices are intended to meet the Mental Health and Substance Abuse needs of children, adolescents, and adults, as well as their caregivers and supports as appropiate by providing a variety of interventions. These interventions include:

• Linking the consumer with services and supports in his/her community.
• Monitoring and coordinating services provided to a consumer.
• Skill building in the areas of coping skills, social skills, occupational skills, and functional like skills.

Community Support Services at Family NET assists consumers in moving to different levels of treatment care. This service is designed to provide interventions that focus on promoting the well being of consumers using a strengths-based model to help consumers achieve the highest level of functioning within the community.

A consumer is eligible for community support services if there are two identified needs, a Mental Health and/or Substance Abuse diagnosis, and has at least one other area of need such s at risk of institutionalization, hospitalization, placed out of home, in recovery, uses multiple agencies, substantiated abuse, neglect, or dependence, physical aggression.
