COMMUNITY SUPPORT SERVICES |
Goal: Children, adults and their families will have access to nurturing, education, support, and treatment services to maximize family functioning.
Outcome: To avoid family disruption in fiscal year 2007-2008, 55% of consumers will not have to be placed out of the home for treatment due to access of supports and services in the community.
Outcome: By June 2008, 60% of community support children and adolescents that are in the home will show improvement after six months of services, of 3 points in at least one of the following areas, social skills, problem behaviors and/or academic performance as measured by the Social Skills Rating System.

Community Support Sercices are intended to meet the Mental Health and Substance Abuse needs of children, adolescents, and adults, as well as their caregivers and supports as appropiate by providing a variety of interventions. These interventions include:
- Linking the consumer with services and supports in his/her community.
- Monitoring and coordinating services provided to a consumer.
- Skill building in the areas of coping skills, social skills, occupational skills, and functional like skills.
Community Support Services at Family NET assists consumers in moving to different levels of treatment care. This service is designed to provide interventions that focus on promoting the well being of consumers using a strengths-based model to help consumers achieve the highest level of functioning within the community.
A consumer is eligible for community support services if there are two identified needs, a Mental Health and/or Substance Abuse diagnosis, and has at least one other area of need such s at risk of institutionalization, hospitalization, placed out of home, in recovery, uses multiple agencies, substantiated abuse, neglect, or dependence, physical aggression.
